This is an ending in the key of D and played over the A(5 chord) and resolves to D. The last slide is a 5 note strum and the way I do it is: place thumb on 8th string and second finger on 4th string. Strum up the four notes with thumb and hitting the 4th string at end of strum and let all ring while doing the 5 position slide.
