This is a JMI Tone Bender Professional MK II OC75 Fuzz Pedal in mint condition.
It is the "Limited Edition", "Gary Hurst" (Inventor of the Tone Bender) Signature pedal, and is a handmade exact replica of the original, down to the NOS Mullard OC75 germanium transistors. It comes with the original box and all paperwork, including the certificate of authenticity.
This is the same pedal used by Eric Clapton, Jeff Beck, Pete Townsend, and Jimmy Page, and it sounds pretty cool on the steel also.
