How would you add a 4th pedal to a Pro 1? There is only room between the blocks on the pedal bar for three.

I'd get a longer axle rod and put another pedal hanging on the end. I don't think it'd harm to put just one on the far side. Or you could:
1) have the existing holes filled (do it yourself if you're a welder, I'm not), and move the block 2.75" over, add a longer axle and put 4 pedals between the blocks.
2) put in a third block and a longer axle.
