I realize this is a long shot, but I know some guys have piles of old guitar crap laying around, and its worth a try.
I have an old Harmony electric,and an old Framus banjo each missing a single tuner shaft ferrule.
The little metal ones that push in, and then fall right back out. 
If you have some, I would be happy to pay whatever you feel they are worth.
I guess I could measure the ones I have, or perhaps you could send several and I could send back what I don't use... something..
