Does anyone recognize this? Model number, year, if it's really a Sho Bud?

It's a Sho-Bud Maverick - student model - most likely from the early '70's.

And it's one with the raised neck and the V-type cast tuning head...that's a keeper!!!

It's on flea-bay for $800.00 with a Peavey LTD 400 amp. It looks like a good deal.

Yep, that's the best playing and best looking Sho-Bud Maverick made. And it's not a bad price for the guitar and amp. It can get you started. However, it is still a student model, and does not have a modern full professional set of levers.
