Randy, if you are only going to need one decal you can buy press-on lettering and borders in many styles at office supply stores. You can then press the lettering directly on the wood if you plan to have a clear finish on the wood and then spray or paint the clear finish coat over the decal.
If you are going to stain the body, do that first and then apply the press-on lettering and finally spray or paint the body with a clear gloss coat. The lettering comes in black or white.
If you want to have a decal that you can paste on the body after you have put the finish on it, you can take a piece of clear tape such as is used for packages and paste it to the slick paper that postage stamps come on (after the postage stamps have been used, of course). Next press on the lettering that you want on this layer of tape and then put another strip of the clear tape over the lettering. Cut the decal to size and strip off the back slick stamp paper. Then you have the original tape sticky side exposed and you can stick it in place on the body.
Even a yet easier way is to select a lettering font on your computer that you find attractive (Fender used the "Brush Script" syle) and then design and type the decal and save it to a file. Buy some clear label paper at Staples, etc. that will print on your computer and print out the decal. Be careful to buy clear labels; some that you think are clear will actually be "snowy" when you remove them from the backing and stick them to the guitar body. This computer approach is especailly good if you want to make several decals, model names, etc.
