Moon River final bars

Hi everybody,

My band wants to play Lloyd's version of Moon River. I can play most of it, but I really really would like to play the ending as Lloyd Green does. From Bb7 to F#7 and then somehow back to Ebmaj7 with the chimes. But I have no idea how Lloyd does this (familiar question, right?;-)

Can somebody give me a direction please?
