I know there aren't many of these beasts in captivity, but I'm curious to know what folk do with them. I imagine that one neck would be extended E9, which is kind of standard, but what do you do with the extra strings on the C6?

I don't play a D-12, but the one I played was set up like this from top to bottom:
D-B-G-E-C-A-G-E-C-A-F-C

I tuned the front neck to an E9/B6 Universal tuning...without the usual D string. I only used four pedals on that neck. On the C6th neck, I used the standard C6th tuning, but added a B and a D out in front of the standard 10 strings. It sort of gives you the "chromatics" found on the E9th neck that way.

I have been playing a D12 since about 1966 and I use extended E9 and C6 variation as follows top to bottom
D G E C A G E C A F C F

Comparing all the answers, including the ones on b0b's site, it seems that everyone's first choice is to have a high G and a D, instead of having to choose - no surprise really. After that it's a question of whether to imitate the E9 with an additional B, or to fill in with a low D, as some folk feel short of strings down there.
Obviously no-one would want anything lower than the bottom C - and then I see what Robbie's been doing all this time! That must be a monster sound

I have extended E9 on the front neck with a low G# and E. On C6 my first strings are F, D, G, E, C, etc. down to the low C on the 12th string. The F and D on top are similar to the top chromatics on E9 and I keep both the D and G of 10 string tunings.

Going off the C6 topic very briefly, b0b - I like your LKV on the E9 - much more elegant than the full C pedal.
