You (or get someone who can) make one using a neck selector switch, three jacks and a small metal box from Radio Shack. Make sure the switch is a make -before - break type to avoid any pop when switching.
Put two jacks on one side of box for inputs to switch and one jack on other side for the output.
(Just pretend the box is a steel guitar with two pickups and a neck select switch that sends the signal to the output jack. Might cost you a few bucks for parts, but it works!
